Barnardo’s – Moving Forward Health

Moving Forward Health supports young people with health conditions or disabilities to transition into adulthood. The focus is to enable them to take greater control and independence of their health as they move into adulthood. This service is commissioned and free to use for service users. This service is provided by a voluntary or third sector organisation.
